Insurance & Payment
Alan M. Lazar, MD, FACS accepts most insurance plans, HMO/PPOs, Medicare, Workers' Comp, and most major credit cards. We understand that insurance requirements and claim procedures have become increasingly complicated. Our friendly office staff is ready to assist you in processing your claims. Co-payments, as mandated by your insurance, are required at the time of service.
